Attendance/Important Documents

• To submit an attendance document electronically - Educational Enhancement Opportunity Form, Parent Note, Doctor's Note, etc...- please email that document to Megan Mattox at [email protected] 

• Information Sheet (Important contact information, kept on file in case of emergency)
• College Visit Form (Also referred to as Educational Enhancement Opportunity Form.  Must be filled out at least three days before planned visit and approved by Principal)
• Educational Field Trip (Also referred to as Educational Enhancement Opportunity Form.  Must be approved by Paris School Board prior to any family trip deemed to be of educational value)
• Kentucky High School Athletic Association Physical Form (All students participating in athletics are required to complete this physical form.)

Student Drivers

• Parking Registration Form (For licensed drivers only.)
• Parking Policies (For licensed drivers only.)
• Driver Permit Form (This form must be picked up in person from High School Secretary two days prior to driver permit test.  Student must be passing five of seven classes and have no more than nine unexcused absences.)
